Our Services

There are generally four kinds of services available to help seniors and caregivers:

Services in Your Home

Friendly Visiting

Regular visits from a trained volunteer to a senior in their home, to provide companionship and social support.

Home Help or Homemaking

Assisting a senior at home with routine household activities including light housekeeping, laundry, and light meal preparation.

Home Maintenance and Repair

Repairs and maintenance to a senior’s home, performed regularly, occasionally, or just once.

Meals on Wheels

Nutritious meals delivered to seniors at their home. Meals are delivered by volunteers or staff who provide seniors with regular social contact and check on their safety.

Personal Care or Personal Support

Helping seniors with personal hygiene and other activities of daily living.

Respite

Support services for seniors, to temporarily relieve their caregivers.

Security Check

A regular phone call or face-to-face visit in a senior’s home to check on their safety and well-being.

Shopping List Pickup

Pickup of grocery items on a senior’s behalf, and delivery of the items to their home.  Seniors are responsible for the cost of shopping items.

Services in Your Community

Adult Day Program for Frail Seniors

Supervised and supported social and recreational activities for seniors who are physically frail or have challenges, at a location outside their home. Programs include meals, transportation to the program, and some personal care.

Adult Day Program for Seniors with Alzheimer’s Disease and/or Dementia

Supervised and supported social and recreational activities provided at a location outside a senior’s home. Programs include meals, transportation to the program, and some personal care.

Adult Day Program with Overnight Stay for Seniors with Alzheimer’s Disease and/or Dementia

Supervised and supported overnight accommodation at a location outside the senior’s home. Service includes personal care, meals and appropriate activities.

Group Dining

A nutritious meal for seniors, provided at a central location on pre-arranged days.

Shopping Assistance

Escorting seniors to a store and helping with the purchase of groceries or medications. Seniors are responsible for the cost of shopping items.

Shopping Trips

Transportation for groups of seniors from their homes to designated neighbourhood stores or malls and back home. Seniors are responsible for the cost of shopping items.

Social and Recreational Activities

Social and recreational activities for a range of seniors, from those who are very healthy, to those needing many services to remain living independently.

Transportation

Rides for seniors who are unable to use public transportation and are not eligible for “Wheel Trans”, York Region Mobility or other municipal transport programs.  Rides are provided to medical or therapy appointments, shopping and various social activities and programs. Transportation is provided by staff or volunteers.

Counselling, Support and Education

Case Management

Conducting an assessment, and planning and coordinating services to meet a senior’s needs, goals and expected outcomes. Service may be provided at a senior’s home or another location.

Crisis Support and Assistance

Support and assistance for a senior in a crisis situation (e.g. an abusive situation), including planning and coordinating other services.

Health Promotion and Education

Education and knowledge to seniors, caregivers, and the broader community about providing care to seniors.

Hospice Care

Information, education and support (physical, practical, emotional, social, spiritual, and bereavement) for seniors living with a progressive and life threatening illness, and for their caregivers, families and friends.

Respite

Support services for seniors, to temporarily relieve their caregivers

Social Work

Trained professionals providing support and counselling to help seniors, their families and other individuals to develop the skills and abilities to function as independently as possible. 

Support for Individuals or Families Caring for a Senior

Support provided to individuals and/or family members who are providing care and support for a senior. 

Support Groups for Individuals or Families Caring for a Senior

Support for groups of individuals and/or family members who are providing care and support for a senior. 

Supportive Housing for Seniors

Designated buildings with on-site personal care and support services for seniors.

Health Care and Specialized Services

Foot Care

Helping a senior with the care and condition of their feet including monitoring for health conditions. 

Hospice Care

Information, education and support (physical, practical, emotional, social, spiritual, and bereavement) for seniors living with a progressive and life threatening illness, and for their caregivers, families and friends.

Nursing

Health promotion, assessment, care and treatment of health conditions to reach or maintain optimal health.

Palliative Care

The combination of therapies, such as nursing and social work, delivered by a multidisciplinary team, intended to comfort and support individuals and families who are living with or dying from a progressive life-threatening illness.

Rehabilitation

A variety of therapies and /or education, such as occupational therapy and physiotherapy, to reach and maintain optimal physical, emotional and psychiatric health and independence.

Specialized Geriatric Services

A variety of health care services that diagnose, treat and rehabilitate frail seniors with complex medical, functional and psychosocial issues.  Services provided in the senior’s home, in inpatient, or clinic /outpatient settings by a team of health professionals including geriatricians, nurses, social workers, and occupational and physical therapists.
